Dental Device or Pillar procedure which to try first

Post Dental Device or Pillar procedure which to try first 
just received my sleep study back. AHI of 16. My ENT says I'm a good candidate for Pillar (small neck, tongue doesn't appear to be the problem, believes its mainly soft palate issues. I have read quite a bit on forums and release most people only have about a 50% reduction and often have to use the procedure in combination with a dental device to achieve less than 5 AHI status.

My question is going by what the ENT says about my tongue, should I look towards Pillar first or a dental appliance first. I do have an certified sleep medicine dentist in my area. My guess is insurance will not cover either of the choices so where should a I throw my money first? Thanks

I have had over 3 years of success with my SUAD dental appliace. My insurance covered it because I was diagnosed with OSA. I don't have much knowledge of the Pillar procedure, but thought I'd give my 2c. :)
