With a pressure setting of 18, you might need something more than basic CPAP, bearing in mind the top pressure for CPAP machines is 20.
From what you say your family doctor might not be the best person to advise you. Can you contact the clinic where you had your sleep test ?
My first pressure setting (back in 1994) was 18, and it could nearly lift the mask off my face. The problem with this is that if you move your head at all while asleep you usually get leakage. Thankfully this type of problem can now be addressed with BiPAP, CFlex and even auto adjusting PAP.
I think you need to deal with a sleep professional. Explain your problem to your doctor and have them refer you back to the sleep clinic.
It is vitally important that you become compliant with cpap and the first 6 weeks are crucial.
