Thanks to all for your posts.  I recently 10 days ago received the Silencer Pro to treat my mild OSA (RDI 9.6 in all positions, 19 in supine...all in REM).  I think these numbers are pretty low, the docs think they might skew a bit higher since I didn't have great sleep efficiency in my two sleep labs and hence not too much REM.  The real problem is that I feel like *&^^%!  CPAP sucked for me and I did try...kept it on most of the night, but doubt I was getting any real deep sleep so I felt worse.

Sorry for all the data, but I'm sure some people will ask (Kent Smith, I hope you're reading:).   My Ortho told me it could take up to 2 months to really feel the effects of this device.  I think he meant it would take that long to dial it out to the proper setting, but shouldn't I feel better immediately once my throat is open to the correct space?  Here's where my question comes in:  I felt better (not excellent, but better) right away at the 2nd setting.  Since I didn't have any problems, I upped it to the next setting and then the next (only one more setting to go now) and I feel worse.  I have since moved the settings back to the middle.  Does OA therapy take time to feel even if it's titrated right?  I know there's no guarantees here, but I've spoken/read a lot on this and I should be a great candidate for OA.  BTW, I'm totally healthy in other regards (blood test, etc to confirm)...just this relentless tiredness/sleepiness/brain fog!  

My theories:
A) its working and I'm paying off a sleep debt and will feel worse before better (doubt this can even happen, but really hoping that's the answer)

B) it's over titrated (doubt this as well as I think the throat is like a valve and the more open the better...right)

C) it's not working and the last 2MM move probably won't work better

D) maybe it's working but I'm still going to be sleepy for awhile?

Do I need a new device?  Do some OAs really push the jaw out and the Silencer is more conservative?  If there's no relief on the Silencer, will I get any relief from another one of these?

I live near Stanford and am talking to these guys about Somnoplasty and would be completely happy to do that and have a device.
