Trying to do some research.  If you have any experience with having varicose veins treated, would you share?

1.  What kind of treatment (Surgery? Laser? Radio-frequency ablation? Other?)
2.  Did it work?
3.  Did insurance cover it?  If yes, what did you have to do to convince them it was medically necessary?
4.  What did it ultimately cost you out of pocket?
