This site claims to have every physician in the U.S. listed. You can rate your physician using a 0-10 scale rating system and also leave free text comments. It's anonymous so you can be completely honest, and the purpose is to allow the physicians to access the data to find out how their patients feel about their services, office staff, etc.

Who knows if any of them actually look at it, but I rated my docs and it felt good to say what was good and what needs improvement.
