In order to find your optimal position you should really get a titration study done.  If this is not possible you may try using a pulse oximeter to record your oxygen levels at night.  If your oxygen levels remain high, you’re dreaming all night and you feel more refreshed then this may be an indication that you’ve reached your optimal position.  Personally, it took me 3 months of slow adjustments to get my jaw to protrude to 5.25 mm – my setting.  I read somewhere that most people will have noticeable improvement after 5 mm.  Once your jaw starts getting tight try to leave it a week at each setting so you can feel the results.  You’ll know when you’ve reached your max point – your jaw is so tight that if you try to adjust out any more you’ll feel pain.  My optimal point was 2 turns less then the max point – about .5 mm less than the max (I have the Tap 1 where one turn is .25 mm)  You’ll know when you’ve reached optimal point because you’ll start dreaming all night and wake up more refreshed – without that “angry” feeling.

phoebe do you mean 5.5 mm past nuetral(were your top and bottom teeth are even). i have an over bite of about 2 to 3 mm.  so if i am now about 1 and a half mm past nuetral, that means i am advanced 3.5 to 4.5 mms.  no pain yet.

Neutral setting would be your normal bite position – your jaw has advanced 0 mm.  I have advanced my jaw 5.25 mm forward from my normal bite position.   It sounds to me like 3.5 -4.5mm in 2 weeks is advancing way too quickly, this may give you jaw problems.  What device do you have and what directions did your dentist give you?  You typically want to advance .25 mm forward once every 3-4 days.  That’s why it takes approx. 2-3 months of patient, slow adjustments to reach your max.  This website www.talkaboutsleep.com has more discussions about oral appliance on their boards just to let you know.
