Hello, I have been on a Bipap since 2000. I have had my machine adjusted many times and I even had another sleep study in the summer. And my machine was adjusted two times after that. Now I can't stay awake during the day.
Does anyone have any info?? I am having another sleep study as soon as they call me and set it up.

Hi, April!

I have been on bipap since '03 and and the only time I remember not feeling sleepy during the day was right after I started using the bipap. There was a brief period where it seemed to be helping but then it wore off. I didn't go back to the doc in '04 and just recently went back for a check up.

This was the game plan for fixing the continued sleepiness:

Re-titration to see if the machine settings needed to be changed
Go back on Trazodone to get insomnia under control - more sleep at night might mean less sleepy during the day

My doctor has mentioned meds a couple of times; the kind that help you stay awake during the day. But he says it's not a good thing to do in my case since I have insomnia and because then you get into that cycle of needing something to stay awake and then something to fall asleep.

My new titration study was not what I was expecting and things are sort of unsettled at the moment as far as where to go with treatment. While waiting for the latest results, I've started back on the Trazodone and last night had the best night of sleep I've had in a long time. But this morning I had a hard time waking up and felt doped all morning and still sleepy after the drugged feeling wore off.

If you find a cure for your daytime sleepiness please let me know. Sometimes I'm so sleepy it's almost painful.

You need to talk to your doc about a MSLT, or PVT test, or Maintenance of Wakefulness Test.  If you are having daytime sleepiness it is quite possible that you need to be medicated to resolve that (do a search on Provigil)

Post still sleepy on BiPAP 
Have you been ruled out for Central Sleep Apnea?  Spontaneous mode bipap (the most common machine ordered) doesn't treat central events.  You need a backup breath rate to be sure the central events are treated.  This could explain why you're still tired.
