It seems to me that a dental appliance plus sleeping upright in my recliner may be the best way to go. AHI of 88----no REM sleep on 1st test. But then again, they did force me to lie down-----So naturally the results were bad.
Based on scientific/medical evidence, this is unlikely to work. Severe OSA requires that the airway is splinted open to allow normal breathing.

What I am saying is that I can't see how CPAP can possibly work for me. It seems that my face changes drastically when I truly relax---therefore anything that fits even when I am semi-relaxed won't fit once I am truly asleep----------This doesn't seem to be a situation that can be remedied.
The use of a double skinned mask, which allows the cushion to be inflated is usually the best bet for maintaining a seal, particularly where patient movement is involved.

What I haven't experienced is having a skilled dentist inventor willing to work on my case. I've consulted 20 "experts" who weren't willing to attempt therapy----now I finally have one!
20 say no, and you find one who says yes..........does that not give you an indication of the type of person you are about to deal with. He wants a guinea pig. You are it. I do wish you well. He is going against all the studies, percentages and numbers.

My dentist is a real pro and insists on at home tests before and after the device is fitted------He wants to prove that special cases can be helped substantially by dental appliances---
He is certainly not a professional. Don't kid yourself there. He wants to prove something.......this being the case, you are not a patient, but a participant in his experiment............which, at present flies in the face of proven studies.
On the other side, you are an adult and can make your own decisions.
Best of luck to you.
Daniel.
